The plans and programs outlined here are designed to support our associates’ physical, mental, and financial well-being, and the personalized enrollment experience was designed to be easy to use. Please take some time to learn about what’s available and how you can help your associates make the most of their benefits.
- Provide the Marketplace Notice within seven days of hire.
- Showcase our benefits by walking through the digital tools available:
- Benefits Overview – a quick summary of Walmart benefits
- One.Walmart.com/BenefitsGuide – a personalized guide to the benefits each associate is eligible for, enrolled in, and able to use
- One.Walmart.com/Wellbeing – benefits and programs for the mind, body, and wallet
- If you’re onboarding a new associate, walk them through one of the benefits onboarding decks, found in the Onboarding tab.
- Help with eligibility questions. Eligibility is based on associate classification and hours worked, so eligibility details can vary. Please refer associates to the benefits information they’ve received in the mail, or to “Helping with eligibility questions” under the Onboarding tab. Associates can also check their eligibility by contacting People Services.

Below you’ll find the 2025 benefits available for your facility. Keep in mind that not all associates are eligible for all programs.
For medical-eligible associates in parts of FL, IL, IN, OK, and TX:
Get the same coverage for all network providers
Now you’ll get the same level of coverage when you see any health care provider in your plan’s network. You’ll have help finding quality network providers using transparent ratings based on provider history of proper care procedures and better patient health outcomes at IncludedHealth.com/Walmart.
For medical-eligible associates in parts of northwest Arkansas:

For medical-eligible associates in parts of AR, IL, IN, OK, and TX:
Coverage changes for your plan
This year you’ll have coverage when you visit a network provider, or out-of-network provider. However, you’ll receive a greater plan benefit when you see a network provider.
For medical-eligible associates in most other areas:

Your plan’s updated provider directory includes transparent ratings based on provider history of proper care procedures and better patient outcomes. You can still choose any network provider, but be sure to check out how they measure up at IncludedHealth.com/Walmart.
For associates who are enrolled in the PPO Plan:
Heads up: PPO Plan going away in 2026
Your current PPO Plan is still available to you in 2025 because you’re already enrolled. However, the PPO Plan is now closed to new enrollments and will be discontinued in 2026.
For associates enrolled in national plans in parts of MN, NC, and NV:
Your medical third-party administrator has changed
Your medical plan has a new third-party administrator for 2025. This means the list of network providers in your plan may have changed, although many of the same providers are still available. You can find providers in the new network at IncludedHealth.com/Walmart.
For all medical-eligible associates except those in Hawaii:
Expanded coverage for cancer and heart surgery
If you're facing a recent cancer diagnosis, most cancers now qualify for a diagnosis and treatment review to see if you could benefit from an on-site visit to Mayo Clinic. If you are facing a serious heart surgery, you're eligible for a medical record review, with a possible onsite visit at Cleveland Clinic. We're proud to keep building on the Centers of Excellence program, which connects you with specialists at some of the best facilities in the country for major needs like these. For all medical-eligible associates except those in Hawaii:
Physical therapy at no cost to you
Feel better quicker with virtual physical therapy and get a $0 copay with most medical plans. You’ll get direct access to a licensed physical therapist, a personalized treatment plan, and the freedom to do your therapy anytime, anywhere. Participants enrolled in the Saver Plan must meet their deductible first. Learn more at One.Walmart.com/VirtualCare.
For all medical-eligible associates except those enrolled in an HMO plan.
Personalized support for diabetes
If you have type 2 diabetes you may be eligible for Diabetes and Metabolic Management by Twin Health. This program can help you reach your health goals using a digital twin of your metabolism to deliver personalized recommendations for nutrition, activity, sleep, and more. Connecting associates with their benefits
As a manager, you play an important part in helping connect associates with the benefits and programs that support their overall well-being. Below is a quick checklist of ways you can make sure associates new to Walmart benefits have the information they need and know where to go to learn more.
- For new or rehired associates only: Provide the Marketplace Notice to associates within seven days of hire.
- Share a benefits presentation and hand out one-pager:
- One-pager (this is a quick reference capturing links and QR codes for full-time hourly associates)
- Introduce associates to use their digital benefits tools:
- One.Walmart.com/BenefitsGuide (a personalized overview of benefits and enrollments)
- One.Walmart.com/Wellbeing (benefits and programs for the mind, body, and wallet)
- One.Walmart.com/Enroll (to sign up for benefits)
Eligibility is based on associate classification and hours worked, so eligibility details can vary. If the associate is eligible and enrolls, they’ll be given the option of covering eligible dependents.
- Spouse/domestic partner*
- Children and/or stepchildren up to age 26 — This includes natural, adopted, legal guardianship, foster, stepchildren, and/or children of your eligible partner.
Associates are eligible for some benefits on day one, others require enrollment within the first 60 days, and others become effective within 90 days, depending on associate classification. Check out these benefits timelines for an overview:
*Domestic partners must meet eligibility requirements to be covered. Spouse/partner coverage is not available for some benefits for part-time and/or temporary associates.

Associates who are leaving Walmart may have questions about what happens to their benefits after their final day at work.
Some Walmart benefits end on their last day of employment. For other benefits, the associate may elect to continue coverage and make monthly payments.
