Eligibility: These COE benefits are available if you’re enrolled in the Premier PPO, Saver HSA, Contribution HRA, or Personalized Wellbeing Copay Plan.

Eligible full-time associates also have access to benefits for surrogacy and adoption, which do not require medical plan enrollment.

Coverage: With most plans, fertility treatments and services are covered at 75% after you meet your plan’s deductible. With the Personalized Wellbeing Copay Plan, you pay the applicable copays for the services you receive. Covered services include, but are not limited to, in vitro fertilization (IVF) and intrauterine insemination (IUI). These services are subject to your out-of-pocket maximum. Fertility treatments and services, including IVF and IUI, are also subject to a $20,000 lifetime maximum benefit per person. 

Coverage for these services is only available at a COE provider under the COE program, which means your plan won’t cover fertility care you receive anywhere else unless those services are covered under other medical plan terms and conditions. Travel and lodging for you and an adult caregiver are covered if travel is recommended by the COE provider.

Coverage: The COE program covers certain heart surgeries and cardiac ablation procedures. If you’re being recommended for non-emergency heart surgery, you are eligible for a cardiac medical record review by Cleveland Clinic to make sure the treatment is appropriate. If Cleveland Clinic recommends heart surgery or a cardiac ablation procedure, you’ll be eligible to have it performed at Cleveland Clinic at no cost to you with most medical plans.

Eligible services are covered at 100% before you meet your deductible under the Premier PPO and Contribution HRA Plans.  If you’re in the Saver HSA Plan, you have to meet your deductible first. If you’re in the Personalized Wellbeing Copay Plan, eligible services are covered with no copay. Travel and lodging for you and an adult caregiver are also covered if travel is recommended for eligible services at the COE facility.

Coverage: Eligible hip and knee joint replacement services are covered at 100% under the COE program before you meet your deductible under the Premier PPO and Contribution HRA Plans. If you’re in the Saver HSA Plan, you have to meet your deductible first. If you’re in the Personalized Wellbeing Copay Plan, eligible services are covered with no copay.

If you choose to receive hip or knee joint replacement services outside of the COE program without a network exception , the level of coverage depends on your plan:

Premier PPO, Saver HSA, and Contribution HRA Plans: Benefits are reduced to 50% after out-of-network deductible. Coinsurance will not apply to your out-of-pocket maximum. Personalized Wellbeing Copay Plan: Coverage is only available under the COE program, which means your plan won’t cover services you receive outside of the COE program  without a network exception. Travel and lodging for you and an adult caregiver are also covered.

Coverage: Most organ and tissue transplants are covered at 100% under the COE program before you meet your deductible under the Premier PPO and Contribution HRA Plans. If you’re in the Saver HSA Plan, you have to meet your deductible first. If you’re in the Personalized Wellbeing Copay Plan, eligible services are covered with no copay.

Eligible transplant services include:

• Liver
• Kidney
• Heart (including durable ventricular assist devices [VADs] and total artificial hearts)
• Lung (including lung volume reduction surgery [LVRS])
• Pancreas
• Simultaneous kidney/pancreas transplants
• Multiple organ transplants
• Bone marrow/stem cell transplants (including CAR T-cell treatment).


Coverage is only available under the Centers of Excellence program, which means your plan won’t cover services you receive anywhere else unless you apply for and receive a network exception. Travel and lodging for you and an adult caregiver are also covered.

Coverage: Surgery for certain spine conditions is covered at 100% under the COE program before you meet your deductible under the Premier PPO and Contribution HRA Plans. If you’re in the Saver HSA Plan, you have to meet your deductible first. If you’re in the Personalized Wellbeing Copay Plan, eligible services are covered with no copay. The COE program does not include some conditions like scoliosis.

Coverage is only available under the Centers of Excellence program, which means your plan won’t cover services you receive anywhere else unless you apply for and receive a network exception.

Weight loss surgery also has a 12-month waiting period from the date your medical coverage becomes effective before this benefit is available. 

Coverage: Weight loss surgery performed through the COE program is covered at the regular benefit level of 75% of eligible in-network costs after you meet your plan’s annual deductible under the Premier PPO, Contribution HRA and Saver HSA Plans. With the Personalized Wellbeing Copay Plan, you pay the applicable copays for the services you receive.

Coverage is only available under the Centers of Excellence program, which means your plan won’t cover services outside of the COE program.
