Preventive care

You’ll get 100% coverage for preventive care when you see doctors in your network. Preventive care includes things like annual checkups, vaccinations, mammograms, and other routine tests to help keep you well.

Simple copays

Copays make it easy to plan for medical costs. Plus there’s no need to meet your deductible first.

  • $4 for video doctor visits through Doctor On Demand
  • $35 for primary care
  • $75 for specialists
  • $75 for urgent care
Other care

You’ll pay the full cost for any additional care you receive until you meet your deductible. After that, the plan pays 75% of all in-network costs.  

One exception: the Centers of Excellence program provides 100% coverage for many serious medical procedures like spine surgery and heart surgery.

Affordable prescriptions

All generic prescriptions are just $4 at a Walmart or Sam’s Club pharmacy.

Annual deductible

Your annual deductible is the amount you’ll have to spend each year before the plan starts paying 75% of the cost. The amount depends on your plan:

  • Mercy SW Missouri Local Plan, Banner Local Plan: $3,000 for yourself only, or $6,000 for yourself and your dependents
  • Select Local Plan: $2,750 individual, $5,500 family
  • All other Local Plans: $1,750 individual, $3,500 family

Keep in mind that Local Plans with the higher deductibles also have the lowest cost per paycheck—so you’ll save one way or the other.

Emergency room

There’s a special $300 emergency room copay for ER services. You also have to meet your annual deductible before plan coverage kicks in. If you’ve already met the deductible, you’ll pay just the $300 copay.

Your total cost

The maximum amount you can spend out of pocket is $6,850 for yourself or $13,700 for yourself and your dependents. Once you reach this amount, all your in-network visits and other costs will be covered at 100%.


Banner Local Plan, Mercy SW Missouri Local Plan, Select Local Plan:

  • $23.50 per paycheck for associate only
  • $131.00 per paycheck for associate plus spouse/partner
  • $38.70 for associate plus children
  • $153.50 for associate plus family


All other Local Plans:

  • $47.60 per paycheck for associate only
  • $197.80 per paycheck for associate plus spouse/partner
  • $74.50 for associate plus children
  • $233.90 for associate plus family

Keep in mind that the Local Plans with the higher cost also have the lowest deductibles—so you’ll save one way or the other.


You’ll pay more if you and/or your spouse/partner use tobacco products and choose not to enroll in and complete a Quit Tobacco program. For details, see the 2020 rates

When can I enroll?
  • During your initial enrollment period; this is when you first become eligible for benefits, and depends on your job classification. Check your enrollment options.
  • During Annual Enrollment, when all eligible associates can sign up or make changes.
  • When you have a status change event, like getting married or divorced, birth or adoption, or changes in your employment that affect benefits coverage. Keep in mind that you can only make changes that are directly related to the event. You’ll find details in the 2020 Associate Benefits Book.
  • If you’re enrolled in Medicare Part D, you’re not eligible to enroll in any Walmart-sponsored medical plan.

When it’s time, enroll online.

  • Banner Local Plan: Phoenix, Arizona area

  • Emory Local Plan: Atlanta, Georgia area

  • Memorial Hermann Local Plan: Houston, Texas area

  • Mercy Arkansas Local Plan: Northwest Arkansas and Fort Smith; McDonald County, Missouri

  • Mercy Oklahoma Local Plan: Oklahoma City, Ada, and Ardmore, Oklahoma

  • Mercy St. Louis Local Plan: in and around St. Louis, Missouri

  • Mercy Southwest Missouri Local Plan: Southwest Missouri, including Joplin and Springfield areas

  • Ochsner Local Plan: Baton Rogue, New Orleans, and Slidell, Louisiana areas

  • Select Local Plan: Various locations, including Chicago, Illinois; metro New York and New Jersey; and Austin, El Paso, and San Antonio, Texas.

  • St. Luke’s Local Plan: Boise, Idaho area

  • UnityPoint Local Plan: portions of Iowa, western Illinois, and Peoria, Illinois areas
