Your annual deductible is $2,750 for yourself, or $5,500 for yourself and your covered dependents. Other than your copays for office visits, this is the amount you’ll have to spend each year before the plan starts paying up to 75% of eligible costs. If you cover dependents, all covered participants' expenses combine to help meet the family deductible.
There’s a $300 copay for emergency services, in addition to your deductible. If you’ve already met the deductible, you’ll pay just a $300 copay in most cases. If you are transported to the emergency department by ground ambulance, there is an additional $300 copay in most cases, in addition to your deductible.
