Get great eye care with low copays and flexible coverage. You can use the plan at any Vision Service Plan (VSP) network provider nationwide, including most Walmart Vision Centers and Sam’s Club Optical. Before you get service, make sure your provider is in the VSP network.
- Eye exams: $4 copay, once every calendar year.
- Frames: $130 frame allowance, once every calendar year.
- Lenses: $4 materials copay when you buy frames and lenses together.
- Progressive lenses: additional $45 copay.
- Contact lens evaluation: up to $60 copay.
- Contact lenses: $130 allowance, once every calendar year instead of glasses.
Some exceptions and additional charges apply. You’ll find the details in the Associate Benefits Book.
- You can get care at a Walmart Vision Center, Sam’s Club Optical, or VSP network provider. NOTE: Confirm that the provider at Walmart Vision Center or Sam’s Club Optical is a VSP network provider.
- Let them know you’re a VSP member, and present your medical or vision plan ID card if you have one.
- Pay your copay and any other required charges during your visit. Your provider and the plan will take care of the rest.
- If for some reason your benefit isn’t handled automatically, you can file a claim through the plan’s website.
