Short-Term Disability – Basic Plan

• If you’re a full-time hourly associate: Coverage is effective 12 months from your date of hire.
• If you’re a full-time hourly vision center (Walmart) or optical (Sam’s Club) manager: Coverage is effective as of your date of hire.

Short-Term Disability – Enhanced Plan

• ​​​If you’re a full-time hourly associate: Coverage is effective 12-months from your date of hire if you enroll during your initial election period. If you enroll later, during Annual Enrollment or as the result of an election change event, coverage will be effective 12-months from the date you enrolled. of the election change event.
• If you’re a full-time hourly vision center (Walmart) or optical (Sam’s Club) manager: Coverage is effective as of your date of hire if you enroll during your initial election period. If you elect coverage outside of the initial election period, there will be a 12-month waiting period for coverage to begin as of the date you enroll.

You can only add or drop short-term disability during Annual Enrollment or if you have an election change event, like a marriage or a birth.

• In most cases, disability claims must be filed within 90 days. A filing delay could result in the denial of your claim.
• Once you’ve filed your claim, review the initial packet sent by Sedgwick which includes necessary information and instructions for next steps. 

Receive pay replacement following the waiting period

• The basic plan replaces 50% of your average weekly wage for up to 25 weeks after a waiting period of seven (7) calendar days, with no weekly maximum for most associates.
• The enhanced plan replaces 60% of your average weekly wage for up to 25 weeks after a waiting period of seven (7) calendar days, with no weekly maximum for most associates.
• o   If you work in New York, there is a weekly maximum of $6,000.

^_Maternity

• If your disability is due to pregnancy, the maternity benefit  replaces 100% of your average weekly wage for up to nine (9) weeks after a waiting period of seven (7) calendar days.
• If you experience medical complications during pregnancy or post-partum and you have exhausted the maternity benefit, you will then be paid based your short-term disability basic or enhanced plan for an additional 16 weeks.

Coverage is effective as of your date of hire.

• For salaried associates: The short-term disability plan replaces 100% of your base pay for up to six weeks after a waiting period of seven (7) calendar days and 75% of your base pay for up to 19 additional weeks.
• For driver associates: The driver short-term disability plan replaces 75% of your average day’s pay for up to 25 weeks, after a waiting period of seven (7) calendar days.

If you’re a salaried associate and your disability is due to pregnancy: 

• The maternity benefit replaces 100% of your base pay for up to nine (9) weeks after a waiting period of seven (7) calendar days.
• If you experience medical complications during pregnancy or post-partum and have exhausted the maternity benefit, the plan replaces 75% of your base pay for up to an additional 16 weeks.

If you’re a driver , and your disability is due to pregnancy: 

• The maternity benefit replaces  100% of your average day’s pay for up to nine (9) weeks after a waiting period of seven (7) calendar days.
• If you experience medical complications during pregnancy or post-partum and have exhausted the maternity benefit, the plan replaces 75% of your average day’s pay for up to an additional 16 weeks. 

Applicable in California, Colorado, Connecticut, Hawaii, Massachusetts, New Jersey, New York, Oregon, Rhode Island, Washington D.C., and Washington. 

• Disability claims filed in New Jersey​​ and New York must be submitted to Sedgwick within 30 days of the date your disability begins.
• ​​​Disability claims filed in​​​​ Hawaii must be submitted to Sedgwick within 90 days of the date your disability begins​​.​​​​
• ​​If you are an hourly full-time associate and work in California, Hawaii, New Jersey, or Rhode Island, you are not eligible for Walmart’s short-term disability coverage because the state in which you work has legally mandated disability plans. You may still be eligible for the maternity benefit under the Walmart short-term disability basic plan.​​​
• ​​In Hawaii, New York and New Jersey​​,​​ Sedgwick will forward claims to Lincoln, which administers the statutory plans in these states.​

​​If you work in Colorado, Connecticut, Massachusetts, Oregon, Washington ​​​state, or Washington, D.C., you are eligible to participate in a Walmart short-term disability plan. Sedgwick will reduce your disability benefits by the amount you are eligible to receive from your legally mandated benefits.
