You’ll get 100% coverage for preventive care when you see providers in your network. Preventive care includes things like annual checkups, vaccinations, mammograms, and other tests to help keep you well.

Copays make it easy to plan for medical costs. Plus, you don’t have to meet your deductible first.
- $0 for virtual primary care, urgent care or mental health visits with Doctor On Demand by Included Health
- $35 for primary care and mental health visits
- $75 for specialists
- $75 for urgent care
- $300 for emergency services in addition to your deductible
- $300 copay for ground ambulance, in addition to your deductible
You pay the full cost for any additional care you receive until you meet your deductible. After that, the plan pays 75% of eligible in-network costs for most services.
For many serious, complex conditions, Walmart’s Centers of Excellence program gives you access to specialists at some of the best facilities in the country. The program covers heart and spine surgeries, knee and hip replacements, medical records review for most cancer diagnoses, transplants, weight loss surgery, and fertility care. You must use a Walmart or Sam’s Club Pharmacy to fill your prescriptions. These pharmacies offer all generic prescriptions for just $4.
You can also skip the pharmacy and get prescriptions by mail through Walmart Home Delivery Pharmacy. Your cost for a 90-day supply through home delivery is three times the cost of a 30-day supply at the pharmacy. Also, specialty medications are only available through Walmart Specialty Pharmacy.
Getting to a healthier you is easier with your virtual health benefits. You have access to a primary care provider you can see online through Doctor On Demand by Included Health, and they can handle your family’s everyday health needs. They’re also there for you to manage chronic conditions such as diabetes, high blood pressure, and asthma. Plus, your virtual primary care provider can connect you with mental health professionals and more. You can even get many prescriptions with just a video visit. You can also use other app-based programs for physical therapy and digestive health support. Most of these online services are available at no cost to you with the Local Plans.
Your annual deductible is the amount you’ll have to spend each year before the plan starts paying 75% of eligible costs. The amount depends on your plan:
- Banner Local Plan: $3,000 for you, or $6,000 for your family (with the lowest cost per pay period of all the plans offered)
- Mercy Arkansas Local Plan: $1,750 for you, or $3,500 for your family (the lowest deductible)
- If you cover dependents, all covered participants' expenses combine to help meet the family deductible.
There’s a $300 copay for emergency services in addition to your deductible. If you’ve already met the deductible, you’ll pay just the $300 copay in most cases. If you are transported to the emergency department by ground ambulance, there is an additional $300 copay in most cases, in addition to your deductible.

- During your initial enrollment period. This is when you first become eligible for benefits, and it depends on your job classification. See your personalized eligibility information at One.Walmart.com/BenefitsGuide.
- During Annual Enrollment, when all eligible associates can sign up or make changes at One.Walmart.com/Enroll.
- When you have an election change event, like a marriage or divorce, a birth or adoption, or a change in your employment that affects benefits eligibility. Keep in mind that you can only make changes that are directly related to the event. You’ll find details in the Associate Benefits Book.
- If you’re enrolled in a Medicare prescription drug plan, you’re not eligible to enroll in any Walmart-sponsored medical plan.
