  • Frames: $130 frame allowance, once every calendar year.
  • Lenses: $4 materials copay when you buy frames and lenses together.
  • Progressive lenses: additional $55 copay.
  • Contact lens evaluation: up to $60 copay.

  • If you’re enrolled in both a Walmart medical plan and the vision plan, you’ll find VSP contact information on your plan ID card.
  • If you’re enrolled in an HMO or have no medical coverage through Walmart, you’ll get a separate VSP ID card mailed to your home.
  • You can get care at a Walmart Vision Center, Sam’s Club Optical, or VSP network provider. NOTE: Confirm that the provider at Walmart Vision Center or Sam’s Club Optical is a VSP network provider.
  • Let them know you’re a VSP member, and present your medical or vision plan ID card if you have one.
  • Pay your copay and any other required charges during your visit. Your provider and the plan will take care of the rest.
  • If for some reason your benefit isn’t handled automatically, you can file a claim through the plan’s website.

Many HMOs offer their own vision coverage. If yours does, compare the cost and benefits before you decide whether to also enroll in vision coverage.
