Ready to sign up? Here’s how:
  • Pick a plan. Log into the enrollment portal, select Long-term disability coverage, and pick the plan that’s right for you.
  • Check your rates. You’ll see your cost per pay period once you make a selection. It’s based on your age and the plan you pick. You can also check your rates on Benefits Rates.
  • Drivers will also need to answer a few questions. As you enroll, you may be asked to fill out a quick health questionnaire, also called “Proof of Good Health.”
    • If you’ve just become eligible for benefits, you get to skip this step.
    • If you have any questions about the questionnaire, check in with Lincoln at 1-877-353-6404.

If you enroll when you first become eligible, coverage begins on your 12-month anniversary for full-time hourly associates and on your hire date for salaried associates.


If you enroll after you first become eligible, your start date for LTD coverage will depend on when you enroll (due to an “election change event” or during Annual Enrollment) and your employment details. Following an election change event, coverage will start the first day of the pay period after you enroll. If you enroll during Annual Enrollment, coverage starts on January 1 of the plan year. If you’re an hourly associate, coverage will not start before your 12-month anniversary of employment.


If you become disabled within the first 12 months after your coverage becomes effective, and you were treated for the disabling condition in the three months prior to electing coverage, you may not be eligible for benefits. If you increase your coverage and become disabled within the first 12 months after the increase becomes effective, and you were treated for the disabling condition in the three months prior to electing coverage, you may not be eligible for the increased benefit amount.

If you enroll after you first become eligible, you’ll be asked to provide a quick health questionnaire called a “Proof of Good Health” form. Lincoln will review this form in the approval process. Coverage will start after Lincoln approves coverage. The start date of your coverage will depend on when you enroll (election change event or Annual Enrollment) and your employment details.


Following an election change event, coverage will start the first day of the pay period after the date your approval is received from Lincoln. If you enroll during annual enrollment, and coverage is approved, then coverage starts on January 1 of the plan year or, if received after January 1, on the first day of the pay period after the date your approval is received from Lincoln.

  • Your cost is based on your age and income. You’ll find your cost in the current Benefit Rates.
  • Your cost per pay period is deducted from all eligible wages, including bonuses.
  • You’re not required to pay for long-term disability from your disability payments. But, if you receive any other payments through Walmart payroll, long-term disability premiums will be withheld.
  • You'll find complete details in the Associate Benefits Book.
  • As you near the end of the short-term disability benefit period, if you have enrolled in long-term disability (LTD) coverage, Sedgwick will refer your file to the LTD carrier, Lincoln Financial Group (Lincoln), to initiate the LTD claim process.
  • A representative from Lincoln will reach out to assist you with your application for LTD benefits. Please be sure to respond to their request to prevent delay with ongoing claim processing.
  • Once your claim has been filed, Lincoln can help you with questions about your LTD claim status or benefits (including payment timelines). You can reach out to them at 877-353-6404.
  • Your LTD benefits are based on the LTD plan you selected during enrollment.
    • The LTD plan pays up to 50% of your average monthly wage for as long as you’re disabled, up to your Social Security retirement age.
    • The Enhanced LTD plan pays up to 60% of your average monthly wage for as long as you’re disabled, until your Social Security retirement age.
    • LTD benefits may be adjusted to work together with other benefits like Social Security.
  • Approved LTD benefits are paid biweekly and continue as long as you remain disabled, as defined by the Plan, or until you meet the plan maximum benefit period.

  • There is LTD coverage specifically for truck drivers, and costs vary from the LTD coverage offered to other associates. See Benefit Rates.
  • Like other associates, truck drivers can choose the 50% LTD plan or the 60% Enhanced LTD plan.
  • Drivers can also choose the maximum period for which LTD benefits may be paid:
    • For up to five years of disability, or
    • For the full duration of your disability until your Social Security retirement age.    

If you enroll or increase your coverage outside of your initial enrollment period, an extra step is required. You’ll be asked to complete a Proof of Good Health form, that Lincoln will review. Your coverage will be subject to approval by Lincoln.


If you transfer to a Truck Driver role

  • You can make changes to your LTD coverage elections within 60 days of the start of the pay period in which you become a driver. Chat with a People Services Representative at One.Walmart.com/BenefitsChat or by calling 800-421-1362.
  • If you are enrolled in LTD coverage on the date you become a driver, you'll be transferred to the same level of coverage (50% or 60%) for truck driver LTD at the five-year option. You can:
    • Continue at your current level of coverage (basic or enhanced) with the five-year option
    • Continue at your current level of coverage with the full-duration option*
    • Change your coverage level, and select the five-year or full-duration option*
    • Drop coverage.
  • If you aren’t enrolled in LTD coverage on the date you become a truck driver and want to add it, you will need to enroll in truck driver LTD. You may be required to provide Proof of Good Health if this is not your first chance to enroll in LTD coverage.

*If you miss the 60-day window and later increase your coverage level or duration, Proof of Good Health may be required.

  • You can enroll during your initial enrollment period. This is when you first become eligible for benefits and it depends on your job classification. If you've just become eligible for benefits, check your personalized Benefits Guide to see your enrollment deadline.
  • You may still enroll after your initial enrollment period during these timeframes:
    • During Annual Enrollment, when all eligible associates can sign up or make changes.
    • When you have an “election change event”, like a marriage or divorce, a birth or an adoption, or a change in your employment that affects benefits coverage.
