  • Frames: $130 frame allowance, once every calendar year.
  • Lenses: $4 materials copay when you buy frames and lenses together.
  • Progressive lenses: additional $55 copay.
  • Contact lens evaluation: up to $60 copay.
  • Contact lenses: $130 allowance, once every calendar year instead of glasses.

Some exceptions and additional charges apply. You’ll find the details in the Associate Benefits Book.

ID cards
  • If you’re enrolled in both a Walmart medical plan and the vision plan, you’ll find VSP contact information on your medical plan ID card.
  • If you’re enrolled in an HMO or have no medical coverage through Walmart, you’ll get a separate VSP ID card mailed to your home.
Using the plan
  • You can get care at a Walmart Vision Center, Sam’s Club Optical, or VSP network provider.
  • When you make an appointment, let them know you’re a VSP member, and present your medical or vision plan ID card if you have one.
  • Pay your copay and any other required charges during your visit. Your provider and the plan will take care of the rest.
  • If for some reason your benefit isn’t handled automatically, you can file a claim through the plan’s website.
Enrolled in an HMO?

Many HMOs offer their own vision coverage. If yours does, compare the cost and benefits before you decide which is best for you.
