How it works

If you're looking for great medical care at a good price, take a look at the Select Network Plan. You'll get personalized, high-quality care with the lowest cost per paycheck of all plans available in your area. This special plan is offered in certain areas instead of the HRA High Plan. Here's how it works.

Local providers. To make great care more convenient, the network is built around a top local health system that can deliver great care efficiently. For most conditions, care is limited to this network.

You'll get a choice of nearby providers for every service you might need. Plus, you can still see a doctor outside the network for medical emergencies. But if you choose to go out-of-network for non-emergency services, you'll pay the full cost.

Predictable copays. To make managing your costs easier, the plan features flat copays for visiting the doctor: $35 for primary care, and $75 for specialists. And with copays, there’s no need to meet your deductible first.

Pharmacy and more. The plan covers prescriptions only at Walmart and Sam’s Club pharmacies—and by mail order for prescriptions you take regularly—with generics for just $4. Specialty medications are available only from Walmart Specialty Pharmacy and ESI/Accredo Specialty Pharmacies. And as with our other plans, you’ll have access to programs like Doctor On Demandd™, Grand Rounds, and Centers of Excellence.

Your share of the cost for coverage depends on which family members you cover. If you and/or your covered spouse/partner use tobacco products and choose not to enroll in and complete a Quit Tobacco program, you'll pay more for your medical coverage. Plan details

  • Primary care physician office visit: $35 copay
  • Specialist office visit: $75 copay
  • Other services, including inpatient and outpatient surgeries: 75 percent coverage by the plan after meeting your deductible
  • Annual deductible: $2,750 for associate only, $5,500 for associate plus dependents
  • Annual out-of-pocket maximum: $6,850 per person, with $13,700 family maximum
  • Centers of Excellence: 100 percent (except weight loss surgery, which is covered as a regular benefit.)
  • Unlike with Walmart’s other medical plans, you must use a Walmart or Sam’s Club pharmacy even if your work location is more than five miles from a Walmart or Sam’s Club pharmacy location in order to receive full pharmacy benefits.

Please note: If you are enrolled in Medicare Part D, you are not eligible to enroll in any Walmart-sponsored medical plan.
