You can use this benefit for things like routine check-ups, diet and nutrition advice, emotional and mental health visits, management of chronic health conditions, and more. If you’re enrolled in the Personalized Wellbeing Copay Plan, you can also see your network primary care provider virtually at no cost to you.

Managing an ongoing condition


In certain states, if you’re living with a serious, chronic condition like diabetes, high blood pressure, or asthma, your virtual primary care provider can connect you with Doctor On Demand by Included Health’s in-house Care Team. This includes doctors, nurses, and nutritional and diabetic health counselors, as well as other kinds of specialists and helpful resources. This coordinated, long-term approach can help you manage your illness.
