This plan gives you and your loved ones financial protection if you suffer certain types of injury or die in an accident. You’re covered whether it happens at work or not.

To make sure your benefit is paid according to your wishes, you must name one or more beneficiaries. They’ll receive the full amount of the benefit if you die​​​​​.​​ ​​​​I​​f you’re injured, you could be paid​​​ 25% or 50% of the benefit. Plus, there ​​​may be ​extra benefits ​​​to help pay for​ education​,​ ​​childcare, and rehabilitation costs.

You can enroll in coverage for just yourself or for you and your eligible dependents. No Proof of Good Health is required for any amount of coverage.

Hourly associates can choose coverage including: $25,000, $50,000, $75,000, $100,000, $150,000, and $200,000.

Salaried associates or full-time truck drivers can choose additional coverage: $300,000, $500,000, $750,000, and $1 million.

Part-time and temporary associates can cover eligible dependent children but not a spouse/partner. All other eligible associates can cover dependent children and a spouse/partner.

If you choose to cover eligible dependents, benefit amounts for your family members are based on the coverage amount you chose.

• AD&D insurance is available to all associates who are eligible for medical coverage, or part-time or temporary associates who have been employed for at least 52 weeks.
• You can choose to cover:
  • Only you
  • You and your dependents (only full-time and salaried associates can cover a spouse/partner)
• No Proof of Good Health is required.
• Benefits are paid directly to you, or to your beneficiary if you die. If you don’t choose a beneficiary, payments will be made to your surviving family members.
